Use this request form to add and/or drop SMHS MD courses (Registration Form 2300-AD).
Required Approval/Permission Courses: For courses that indicate approval/permissions is required (check the SMHS MD Online Catalog here), you must upload email approvals from the course director and/or course coordinator for the course(s) you wish to add/drop. Email approvals must clearly state that you are approved to add/drop course(s) for the specific weeks in your submitted request.
Extramural Electives / Away Rotation Courses: Adding/dropping an extramural elective (or away rotation course) requires submission of a different form available on the MD Registrar's webpage (online here).
Waitlisting a Course: Please check the availability of a course BEFORE submitting this add/drop form. If this course is at full capacity, please indicate if you wish to be added to the waitlist.
Deadlines:
Adding a Course = once the lottery results/registrations are available, students may submit an add/drop form to add a course up until one week before the class begins. Requests to add a course within 30 days of the scheduled start date require Course Director approval. This approval may be submitted with your add request as an attachment.
Dropping a Course = Students may only drop a course up until one month before the scheduled start date.
Requests to drop a course within 30 days of the scheduled start date require Course Director or Course Coordinator approval.
Requests to drop a course within one week of the scheduled start date require Course Director/Coordinator and an OSA Advisory Dean approval. You must provide the course director/coordinator approval to your OSA Advisory Dean FIRST, then submit both approvals with your drop request as an attachment.
