Remote Access Request

Introduction This form is designed to provide a process to reqest and receive Remote Access to a user's computer. In order to gain access a user must first be on the GW VPN and know the workstation name that they wish to connect to. This is the link to the site that users should visit inorder to connect the GW VPN https://it.gwu.edu/vpn . User's will have to know the name their computers inorder to access their workstation. The name can be found by right clicking on the Computer object found under the start menu and choosing properties, the name is listed under "Full computer name" e.g., SMHSCA-45WKS12.ead.gwu.edu.
Terms of Agreement I will exercise due care and caution to ensure the integrity of the workstation. I have reviewed and will abide by all applicable SMHS (link) and GWU (link) acceptable use policies I understand that this form, with up-to-date signatures, must be resubmitted every time the workstation requires reimaging or other extensive support. I will certify to the satisfaction of SMHS technical support staff that I am in compliance with the terms listed within this document and all referenced documents immediately upon request. This will include periodic inspections of the workstation and its software. CASS reserves the right to revoke Remote Access privileges without prior notification for any reason. All decisions to provide Remote Access privileges to SMHS employees are made on a case-by-case basis.
