Local Administrator Privilege Request

Introduction It is not common practice to provide administrative privileges to users of workstations, however, in some instances, this may be necessary. For example, administrative privileges may be needed to run or update an unusual software package. The user requesting administrative privileges must demonstrate sufficient technical ability and continuing compliance.
Terms of Agreement I will exercise due care and caution to ensure the integrity of the workstation. I will review and abide by all applicable copyright and licensing policies. I have reviewed and will abide by all applicable SMHS (link) and GWU (link) acceptable use policies I will not use administrative privileges to compromise or penetrate other systems on the GWU network. I will not install software without acquiring and demonstrating proper licensing and will be able to provide licensing information immediately upon request. I will not remove, alter, or reconfigure any software that has been installed by SMHS IT support personnel. I understand that this form, with up-to-date signatures, must be resubmitted every time the workstation requires reimaging or other extensive support. I understand that running a workstation with administrative privileges greatly increases the ability for malware and viruses to corrupt a system. I will certify to the satisfaction of SMHS technical support staff that I am in compliance with the terms listed within this document and all referenced documents immediately upon request. This will include periodic inspections of the workstation and its software. CASS reserves the right to revoke administrative privileges without prior notification for any reason. All decisions to provide administrative privileges to SMHS employees are made on a case-by-case basis.
